If you are inquiring today about treatment required as a result of a motor vehicle accident, we are here to assist you.

The first step is to contact our office to book an appointment. Your initial assessment /treatment will be for 1 hour. We ask that you come into the office 10-15 minutes early in order to complete the paper work necessary for your file. Our administrative staff will assist you with this process. We will need the following information:

  • The name of your Automobile Insurance Co. & telephone number
  • The name of your Accident Benefit Adjuster & telephone number
  • The Claim # assigned to your file by your Automobile Insurance Co.

Please note: An Accident Benefit Adjuster & Claim # are assigned to you after the completion of your “Application for Medical & Rehabilitation Benefits” – Form OCF1 which is provided, for your completion, by your Automobile Insurance Company

  • Your personal Extended Health Care plan information. Under the current legislation, your extended plan must be billed first. Reference: Auto Bulletin (website)

After your initial appointment/assessment, the physiotherapist will submit a treatment plan for the cost of your proposed treatment to your auto insurance company as required by FSCO Statutory Accident Benefits Guidelines. Dependent upon the type of treatment plan submitted, the automobile insurance company has up to 10 days to respond to our treatment plan.
